This template I made that is to help guide students to take notes in Math. I use the Four-Step Plan in my lessons (I call it the UPSCheck though) and this note sheet encompasses it. I usually fill in the examples if they are word problems so the students don't have to spend too much time writing the word problems down. I will also add all the notes as a scaffold for some students.
